Before your appointment

• Please do not attend and book a telehealth appointment if experiencing flu symptoms or if you have had a positive RA test.

• Please ensure you bring your own mask.

• Any abuse of any type to reception/doctors/nurses will not be tolerated and you will be reported to the Police - We all deserve a safe work environment.

• Oaktree is a Private billing clinic - Payment is required at the end of the consultation.

Additional Information

• If working on a mine site and declaring a medication/medical condition, please ask your GP to give you a medication declaration letter. These will not be processed after your appointment and do not meet the criteria for a medicare rebate.

Occupational Health Bookings

• Please bring loose-fitting clothing. Skinny jeans etc restrict your movement.

• Bring your manners - we are here to help, we are not trying to fail you! We deserve a safe environment free from abuse.

• Coalboard medical - Ensure you bring your Section 1 or have your employer email this through to medicals@oaktreemedical.com.au
