GP MANAGEMENT PLANS - (GPMP)

What is a GPMP?

A GP Management Plan helps people with chronic medical conditions by providing an organised approach to care. It is a plan of action created between yourself and your GP that identifies your health and care needs, sets out services to be provided to you, and lists the actions that can be taken to help manage your condition. You may have one chronic medical condition, or many complex conditions requiring care from a multi-disciplinary team, for example, physiotherapist, podiatrist, dietician etc.

What is a Chronic medical condition?

A CHRONIC medical condition is one that has been (or is likely to be) present for 6 months or longer. Some examples of chronic conditions include asthma, cancer, heart disease, diabetes, arthritis, and musculoskeletal conditions.

What is a Team Care Arrangement - TCA?

If your chronic medical condition or complex care needs require a multi-disciplinary team, your GP can develop a TCA. This will mean that your GP will liaise with at least 2 other healthcare providers who will be able to give ongoing treatment or services to you to help manage your condition/s.

What are the benefits of having a Careplan - GPMP/TCA?

·         If you have both a GPMP and TCA prepared for you by your GP, you can claim a Medicare benefit for your allied health services. This helps to minimise your out-of-pocket costs (or eliminate them altogether if your provider accepts the Medicare benefit as full payment).  

·         A GPMP can reduce the need for additional consultations throughout the year to manage your chronic health conditions, as Doctors also try to sync in prescriptions, referrals and ordering of tests all in 3 monthly care plan review visits. The Practice Nurse can also provide support and monitoring between visits to your GP.

·         A GPMP/TCA can help you to actively manage your chronic medical conditions, by receiving the treatment and support you require from both your GP and other health care providers, and by supporting you to have control of your health conditions and overall well-being.

What is a GPMP Review?

Once a plan is in place it should be reviewed by your GP every 3 months. This is an important part of the planning cycle, where you and your GP can check that your goals are being met and agree on any changes that might be needed.

Is there an out-of-pocket cost for a GPMP/TCA and 3-monthly Reviews?

The first visit to create a GPMP/TCA is free, and the 3 monthly Care Plan Review visits are only $20 out of pocket, compared to a full-cost GP consultation.
